Ozaukee County 4-H Record Book Order
If printing in color (black and white not reviewed)...
1- Introduction Page - Has member's name and brief information plus a photo titled 'Introduction'.
2- Permanent Summary Pages
a. Project Summary - a history of all projects taken by a 4-Her that only needs to be added to each year titled "Ozaukee County 4-H" but has a chart to list projects at the bottom of the page and states to keep it throughout your 4-H career at the top.
b. Leadership and Achievement Summary - a two-page form that only needs to be added to each year listing club, county, state, and national leadership responsibilities and awards. Titled "Ozaukee County Permanent Record" and also state to keep it throughout your 4-H career at the top.
3- The Ozaukee County 4-H Club Activity Page - a two-page listing of what activities a 4-Her completed. Titled "My Ozaukee County 4-H Activity Program 20__'.
4- The Jay Road Club Activity Sheet - a two-page form listing club activities available below.
5- 4-H Story Pages - a blank form for the 4-Her to describe his/her 4-H year including experiences enjoyed and why, life skills gained or improved, what was learned and accomplished, leadership experiences, hopes for next year.
6- Project Record Page - one form must be completed for EACH project titled "Ozaukee County 4-H" but asks for project goals mid-page.
7- Project Meeting Dates Page - one form must be completed for EACH project and is titled as such.
8- Project photo or related materials -
9- Additional 4-H Information
Additional forms are required for Cloverbuds, Youth Leadership, Horseless Horse, and several animal projects.
All forms except the Jay Road Club Activity Sheet are available at the Ozaukee County 4-H Website link below under "Record Book Pages", one of the top four choices depending on your print capabilities.
